It no longer refreshed websites. More importantly, CTRL-F5 does not work. I re-installed FF and got it working but it recently stopped working, again. I use CTRL-F5 all the time to see style changes on websites and I really need this to work.

If you hit F5, the menu in that bar disappears. Same when you come to that page via the Back-button in your browser. It only happens on Firefox (seen on versions 3-7). Using Ctrl - F5, the menubar reappears.
